It is the policy of CPAP Express, LLC that all information concerning patient service or care should be treated confidentially. The information may include verbal, telephone, cell phone, written, electronoc and computerized information. Each patient will receive at the start of service a HIPAA Notice of Privacy Practices, which describes how we may use and disclose Protected Health Information (PHI). Protected Health Information includes Electronic Protected Health Information. PHI is information about the patient including demographic information that may identify the patient and that relates to the patient's past, present, and future physical and mental health or condition and related healthcare services. PHI may be used and disclosed by our organization, our office staff and others outside our office that are involved in the patient's service/care and treatment for the purpose providing healthcare services to the patient, to pay the patient's healthcare bills, supporting the operation of the organization, and any other use required by law.
